この辺りの順序は手術書によって様々でした。

膵頭神経叢の切離(上図)の際、すでにmesenteric approachで膵頭神経叢第2部がIPDAを含め半分ほど処理されているので、操作が比較的容易だそうです。なるほど。

PDは経験が浅く、手術書をいくつか読んでやっと描けた感じです。
見たことがないmesenteric approachを整理しておきたかったので描きました。

いきなり膵臓に飛びつくのではなく、横行結腸下からSMA/S MV前面に入り、血管処理と郭清を先行します。
