Her entire ribcage/spine are reinforced to hold those ports in place safely. She actually used to be fairly scrawny, but gained a lot of weight shortly after getting the sockets put in (hybrid biology reacting unexpectedly to painkillers). This led to a LOT of strength training.

The Painkiller protagonist definitely looked very different back in 2002: he was a mercernary hired to kill demons called Oscar Herrera. Definitely a From Dusk till Dawn vibe. But the publisher told People Can fly to rewrite the story to better suit "American audiences".
