Heterosteus asmussi, a 6 meter long freshwater placoderm fish from the Devonian. It may have been an ambush predator, spending most of its time close to the sediment.

Yeah the gnathal plates in placoderms are derived from teeth that grow outwards in in a ring instead of in rows
They also have the same structure as their scales proving that teeth evolved from scales
