The actual answer is both, according to available official artworks plus the anime.
Bulbasaur puts party hats, trainers hat, and cosplay headgears on the head, but it wears sleeping hats on the bulb :P

Bulbasaurus was a Permian synapsid from South Africa. It was named after its bulbous skull, however the describers said the similarity in name to a Pokemon 'may not be entirely coincidental'.
