Look at the shaping in the holster. It's a double-barreled weapon. Which leads me to believe it's this weapon shown in the concept art, renders and cutscenes of Uldren from Forsaken.

He's just moved it to his hip rather than wearing it on his back.

Got an idea for a random undead gnome. She's a "surgeon" (truthfully, just a necromancer) for the Forsaken. May or may not actually realize she's undead. Never stops smiling. Its probably best to find another doctor.
