The very first version already had the dead eyes. After that I changed them but she became more...generic in the following versions. I gave her more damage after which compensated, but recently I decided to just go back to it to complete her look.

She also became a bit shorter.

Yeah, I think it has a lot to do with the characters being older + Mikasa having shorter hair, which causes some people to think they have tweaked her design too much. The fact is the design had already been tweaked between S2 and S3 in order to be closer to the source material.

hansel keeps his hair longer with each reincarnation, while allen's hair is shorter as kokutan

hansel's hair is unkempt/messy with an ahoge/cowlick, while allen's is neater and just less chaotic
