See this flashback is probably where Steph lost her eye, you can see one of the lenses of her helmet/cowl is damaged. The wound is still bandaged when she was talking to Dick but when Steph appears in Batgirls & Robin Eternal she wears an eyepatch not bandages.

That's probably the most curious thing about Batgirl & the Birds of Prey. Cass had four appearances in the comic, but never a full-on "Batgirls" moment with Babs. Instead, Cass/Steph had it with the Sirens.

The last time Cassandra Cain appeared in a Batgirl ongoing was a one-page cameo (the Black Mercy "dream" that had the Batgirls teaming with the Blackhawks and Skeets during 1944) in Batgirl Vol. 3 (2011). For an actual "full" appearance, Batgirl Vol. 3 (2009).
