Just finished Ergo Proxy. That is without a doubt, going into my top 5. Some episodes were a slow burner, but overall, very well done. Loved the soundtrack! The cast wasn't too big or too small either. Cyberpunk, philosophical elements, dark/gritty tone + atmosphere.

Something like this, where Sailor Moon is missing her hair ornaments, is an animation mistake. An in between frame used specifically to create a fluid motion in animation is not. Nobody would even notice her fingers unless they watched the segment in slow motion.
