Technically yes, but the 4th color is transparency. Some games got around this limitation though.
In Mega Man for example, his face is a completely separate sprite from his body. This does cause more flicker/slowdown depending on how many sprites are on screen at once, though.

finished this little animation yesterday for my character doing a little prayer/casting a spell. A cleric home-brew based on Reimu Hakurei from Tohou. The sprites are from Antimony of Common Flowers as well.
The timings could be better but I think it turned out pretty good!
