Mermaid Swamp! It's an RPGMaker horror game by Uri. Mostly exploration, a bit of puzzle solving. Rin Yamazaki, a fair-faced and foul mouthed college student and her friends get stranded near a mysterious swamp. When one of them becomes ill they need to search for a cure.

Wasn't gonna change much outside of more tree frogy colors but then gave her a tadpole tail. She magically grew it so she can swim faster thus save people better from the dangers of the swamp!
