Both Seungho and Nakyum don't wear their headbands and it's the first time NK sees SH without his. It means so much because those headbands separated them, denoting their social statuses as a noble and a commoner. When those labels fall away all that is left are two men in love.

My observation was correct: his habit to grab the nobles by the topknot is linked to his trauma. This is seen from SH's perspective. We have to imagine, he's dragged, pushed down by servants; while lying on the ground his head is grabbed by the topknot.
