At some point Cathy went on her life-long journey to find a way to either get enough power to calm brother down herself or put him to permanent sleep/coma.
She, of course, failed, negative emotions took over after decades of searching and her brother took the wheel :)

The parasites reproduce by infecting the host and eventually awakening and transforming the host. The lowest ranks have a hive mind and aren't sentient.
Cathy's brother is of the lowest rank, awakened, sentient but also dormant and doesn't react much thus Cathy's in control.
