Port
Ports are simply getting games to work on newer systems. Almost completely faithful to the original, with maybe some changes in resolution or controls. Minor changes to get the game running on a console, rather than improving the experience.
Remake
Built from the ground up on a new engine. 'Modern' graphics. Often the game will make updates to animations, sounds, gameplay and sometimes even story. Some examples below.
