#OTD in 1819: An early-type balloon crashes in Paris (France), killing pioneer aviatrix Sophie Blanchard. On a display at Tivoli Garden she threw fireworks from her hydrogen-filled balloon, which caught fire and crashed. She was the first-ever recorded female aviation fatality.
#notOTD 04/03/1959: prototype airliner VEB Baade 152 [DM-ZYA] crashes in Dresden (W. Germany). All 4 aboard die. Jet stalled on flight testing, crew unable to recover. Cause traced to "pilot error", later tests noted designs issues with fuel flow. Project was later shelved.
#OTD in 1976: Zagreb Mid-Air Collison (Yugoslavia). British Airways Flight 476 (Trident) and Inex-Adria Aviopromet Flight 550 (DC-9) collide over Zagreb, all 176 on both planes die. Accident was due to a procedural error by Zagreb ATC, undermanned and poorly equipped at the time.

#OTD in 1969: EC-121 Shootdown. A US Navy EC-121 [135749] on a reconnaissance mission is attacked by N. Korean AF Mig-21 fighters and crashes off Chongjin (North Korea). All 31 aboard die, the largest single loss of U.S. aircrew during the Cold War era.
#OTD in 1986: Challenger Disaster: NASA Space Shuttle “Challenger” explodes after liftoff from Cape Canaveral (Florida, US). All 7 astronauts aboard die. Enquiry pointed to the failure of an O-Ring seal on a rocket booster, triggering a catastrophic failure 73 secs after launch.
#OTD in 1982: Air Florida Flight 90, a B-737, crashes in the Potomac River (Washington, US). 78 of 83 aboard died, along with 4 on a bridge the jet hit. Plane crashed shortly after taking off on snowy conditions; crew had neglected anti-ice procedures, causing loss of control.
