04/03/1959: prototype airliner VEB Baade 152 [DM-ZYA] crashes in Dresden (W. Germany). All 4 aboard die. Jet stalled on flight testing, crew unable to recover. Cause traced to "pilot error", later tests noted designs issues with fuel flow. Project was later shelved.

in 1976: Zagreb Mid-Air Collison (Yugoslavia). British Airways Flight 476 (Trident) and Inex-Adria Aviopromet Flight 550 (DC-9) collide over Zagreb, all 176 on both planes die. Accident was due to a procedural error by Zagreb ATC, undermanned and poorly equipped at the time.

in 1961: United Airlines Flight 859, a DC-8, crashes in Colorado (US). 17 of 122 aboard die, along with 1 on the ground. Jet left runway on landing. The undercarriage was torn, starting a massive fire. Factors: asymmetric thrust due to hydraulic issues, crew actions.

in 1969: EC-121 Shootdown. A US Navy EC-121 [135749] on a reconnaissance mission is attacked by N. Korean AF Mig-21 fighters and crashes off Chongjin (North Korea). All 31 aboard die, the largest single loss of U.S. aircrew during the Cold War era.

in 1986: Challenger Disaster: NASA Space Shuttle “Challenger” explodes after liftoff from Cape Canaveral (Florida, US). All 7 astronauts aboard die. Enquiry pointed to the failure of an O-Ring seal on a rocket booster, triggering a catastrophic failure 73 secs after launch.

in 1982: Air Florida Flight 90, a B-737, crashes in the Potomac River (Washington, US). 78 of 83 aboard died, along with 4 on a bridge the jet hit. Plane crashed shortly after taking off on snowy conditions; crew had neglected anti-ice procedures, causing loss of control.

in 1961: United Airlines Flight 859, a DC-8, crashes in Colorado (US) killing 17 passengers and one person on the ground. The jet experienced hydraulic problems in flight and when landing, it left the runway due to asymmetric power when applying reverse thrust for braking.

in 1980: an Air Traders Constellation [N74CA] crashes in Indiana (US) after crew error when dealing with an engine fire. 3 of 8 aboard died, including Herman “Fish” Salmon, former Lockheed Test pilot – tested P-38, P-80, XF-90, F-94, L-188, YF-104, XFV-1 and Constellation.
