In several illustrations of enemas being given to horses, it seems clear that there is a kind of enema bag involved, attached to a nozzle/funnel.

Note that the monkey has two implements: something like a brown turkey-baster and a little black vessel of some sort. There seems to be a cord or tube connecting them.
Biblissima describes this as a "Scène scatologique", which is putting it mildly, imho.
