For the scene where Ichigo given order by Byakuya, i can see Ichigo feels like a failure for not being there when everyone needed him the most.
He wanted to protect a lot of people, but he "failed". Ichigo just being himself & he feels responsible for what happened in SS.
e.g. when Ichigo gave an order for Uryu to protecting Orihime, i can see Ichigo feels so guilty for not be able to protect someone that he vows to protect in the first place.
And it proved in the next panel where he asks for Uryu's life as the price for Orihime's. Wow. Bold.
