I've even got as far as using the rotation of the arm bone to drive a shape key that preserves the arm's volume and blockiness as the elbow bends, but I'm still figuring out that awkward-looking halfway point.

The twist IS an axis, a rotational one, and so are the chunky-smooth and hot-cold tilts. The axises meet at the basis on which the stick pivots.

All of these movements are just rotations around the pivoting point.
