Q3 I agree with everyone about the text being legible, not crowded, etc but another thing you want to consider is how visually appealing the text layout is. Here's some of my older comic pages as examples of what NOT to do with speech balloons.

Q2 I like to add little flourishes to speech bubbles when the sound is coming from an electronic source, or when a character has a particularly ominous voice.

I would chalk it up to these two...Hellspawn (left) has these weird claw weapon things that are a headache to draw and color...

A1: I used to find hands really difficult when I was new to drawing, but after a lot of practice, they became one of my favorite things to draw! More difficult than hands are feet, as others have mentioned.
