For stage 4 i think Vegeta didn't have enough power or he realized that it's useless to go beyond stage 2. But i personally think he didn't have enough raw power. Also in the manga Vegeta doesn't power down like you see in anime but Trunks does state that Vegeta knew.

I think he did as shown in the manga but i think they wasted too much time the first time training in the Rosat to make any real progress. Vegeta and Trunks also stay in base form after the second time Rosat and they also don't use stage 2 after they lost to Cell in the manga.
