I don't think he's really dead. There are too many references to OFA plus the mentions of All Might and Izuku. The most logical thing is that Katsuki's body is adapting to this new power and at the same time it causes him a lot of damage/pain (just like Deku at the beginning)

What is interesting is that Bridget gender is clearly different between JP and EN official site.

In JP site,I see "男の子(boy)" while in EN site,I can see "her".

If this think logically,gender of Bridget might be divided between JP and EN from now on.
