Like, I could potentially remove his nipples so I could keep the nice 'lore-scar' without having to deal with the face comments (it can cost extra to get your nipples after top surgery so that could make sense in canon) but no nipples feels too weird?!

For example, my character Alex wasn't designed with a scar in mind. A friend drew him with a cut through the eyebrow once. So I went with it! I decided it was a backstory element and worked with the DM to flesh that out. With room for the DM to wiggle as they so chose. I loved it
