I start with a coloured sketch, then add tons of shadows. Then I light it up and add texture. As for backgrounds, I just photobash lol
The final stuff is a multiply and dodge gradient, some blur, and a border. I forgot the subsurface scattering though XD

Final result is surreal enough, but the process was stranger. Started with a photobash of CC0 portraits, AI style transfer w/a fleshy painting of mine as style image, painted on top of result, back through AI, painted on top, etc. for 176 generations to end up here.
