The difference is the quality, more sketchy or refined, and if its a headshot, half-body or full body, etc etc. I'll give u an example. Left: Coloured doodle. Right: 2 character's Headshot Commission.

These are more background friendly, and make for better icons since they have better contrast and defined edges.
