There's reflective imagery here as they explain to Tamaki how he has one month to figure out who is Hikaru and Kaoru with an explanation, saying it as if they know it's impossible yet flipping the image represents how they desire the opposite, the impossible to be possible.

Q2 I think a comic artist needs to consider both. If the page layout is confusing it can make the comic difficult, or impossible, to follow. Panel composition can play a part in that, too. ie: The 180 degrees rule.
