SOLDIER is a huge part of Crisis Core, so this design gets a lot more play there. As such we'll turn to it for examples. In terms of design, the belt is black now, with a black trim, and a black SOLDIER logo, which has been redesigned from the classic version, and no rivets.

This design was used in the original game for Cloud and Sephiroth's belts and appears in CG models, though in-game the rivets are never actually added to the belt assumedly due to technical limitations. Even the SOLDIER logo is just a gray square outside the final battle.

While we're getting niche, the FF7 Versus Guide, an unofficial strategy guide, has two distinct depictions of the sword. One has five rivets but only one hole (much like that earlier beta image) while another has two holes but four rivets.

Chances are they saw his Before Crisis art and misinterpreted the back of the sword as a sharp edge. You can also only see three rivets here, which may explain that. Incidentally, we see Zack is using the classic design in BC, but this was before Advent Children so it's not weird
