The Heide Knight Iron Mask comes from a cut variant present in concept art and not from the "red" Heide Knight, as both helmets are completely different.
There's also an unused hooded variant that reminds me a lot of the Fallen Knight from Dark Souls 3.

This is unexpected...
The red phantom found in Things Betwixt is a Hollow with a human face for some reason. Of course I have no clue as to why, but a similar case happens with the Afflicted Graverobber, with the difference being that her head is missing.

Steady Hand McDuff and Blacksmith Lenigrast share a lot more similarities than just being blacksmiths. I wouldn't be surprise if they were the same character at some point .
