They return the cruelness by crushing this girl's heart, while it's not the best way to go about this situation, it's understandable given this is coming from a place where this situation has happened repeatedly and, while they don't show it, they're hurting on the inside.
We see how this issue affects their romantic life. One twin gets confessed to, lies about their identity to see if they'll notice, proceed to ask if it's okay if they're the other twin and then proceed to call them out if they agree. It's quite a cruel cycle to go through.
Ouran High School Host Club episode 20 was great! I liked how they explored the backstory of Hikaru and Kaoru, and how, as twins, they loathed the fact that they were indistinguishable to others to the point where they shut people out and locked themselves in. Here's a thread!

Episode 16 of Ouran High School Host Club just confirmed Hikaru as the best twin! Great episode! I like Hikaru's character flaws and how he comes to realize them in this episode, his apology for being rude to Haruhi's friend out of selfishness felt genuine.
