The Sonic R album cover (dated January 21, 1998) pre-dates Sonic Adventure's unveiling by 7 months. This was Japan's first glimpse of Sonic's redesign.

Mystic Ruins.

Mushroom is undoubtedly Mushroom Hill Zone but that leaves Jungle and Desert, which could easily be AIZ and Sandopolis Zone.
